Not entirely sure if you are looking for a replacement 'illuminator' or the entire lamp unit!
The 'illuminator' looks like a bespoke unit likely unique to that lamp, and if so, only available (but probably not!) from the lamp manufacturer. If it has failed, it may not be the LED's themselves but the driver circuit on the PCB that has failed, again only available (but probably not!), from the manufacturer.
A competent electronics repairer should be able to fix it, and possibly cheaper than a new replacement unit!
Unless you need to match it into a set, then a substitute lamp may be your best option.
Without an external photo of lamp, it's impossible to do a Google photo search for a likely manufacturer, and I'm sure as you've already found, the number does not yield results, beyond a reference to user manual for a Swift Autocruiser which gives no further info.
