Ours is very similar Scampi. There is nobody at our surgery before about 7:55, so ringing before 8 is pointless. You will just get an answering machine that will give you the out-of-hours number to call. Then at 8:00 the phone goes mad! That's why I don't bother and just drive there. All that day's appointments are usually gone by 8:15 though.

We are both retired and available all day most days, but it still doesn't seem to be any easier to get an appointment. I really sympathise with anyone who works!

In my opinion this is why so many people just turn up at A&E, because at least you will get seen, even if it takes a couple of hours waiting. Not that I would do that of course, unless it was really an emergency, but I know many do!

Colin

That sounds fairly typical. It isn't just around this way then.

Getting GP's to join practices, in NE Wales, reached such a low point around 24 months ago. That over a 6 month period, 6 individual GP Practices, handed back their Contracts to the local Health Board (the largest Practice, had 18,000 Patients Reistered).

This resulted in the Health Board, taking over the running of the Practices, amalmagating them into one. With a Central Site, and the other Practice Sites, becoming Satallite Sites. With the majority of Clerical Staff, now located over 6 miles away from the Main Site, in a Non Medical Facility.

On the Day Appointments, are only held at the Main Site, on a turn up and wait basis.

The Practices, are now a "Primary Health Care Service", broken down into Primary Care Teams, consistin of GP's, Team Coordinator, Practice Nursing Service, and Attached Specialist, including a Pharmacist.

The Team Coordinator, deals with a range of non Clinical decisions, via Telephone conversations, email, of Skype conversations.

The Primary Health Care Service, is being rolled out, throughout the Health Boards, area of operation, due to more GP Practices, handing back their contracts. Due to being unable to recruit GP's and other esential Medical Staff.

The Primary Health Care Model, is being watched by Health Boards, throughout the UK. So expect similar to be rolled out throuhout the UK, in the future.

The Primary Health Care Model, is based on Canadian Health Care Practice.

I feel sorry for us and I feel sorry for the doctors, nurses and staff. Too many people, too few surgeries.

The government keep going on about building new houses all over the place, but they never brag about building a new surgery, or a school for all the people that move into the area. It's happening here. New builds everywhere, no places at any of the local schools, no extra medical staff, and we often wait four weeks for an appointment, although our surgery does have a turn up and wait open surgery two or three times a week if it's urgent.

I do think though that the public (myself included) need clarification on who to consult for different problems.
In the old days, you went to the GP for most things, then if you had a pain in your leg or such you went to Casualty, where they would X-ray it.

Our Practice has nurses who you can almost always get an appt with that day. They can prescribe for coughs and colds, injuries, that sort of stuff. And if there are clinics for all the separate 'well' categories, that removes another load of patients. GP's need to be available for medication changes, hospital referals etc.
Our surgery started with a phone 'triage' system, but it didn't seem to work, but that is what is needed, a professional to feed the callers to the appropriate consultation.

In my day doctors used to come out on house visits. I don't ever remember visiting a doctors surgery as a kid. In fact I am not sure if doctors actually had surgeries in those days. Nurses used to visit schools to give injections for the various illnesses of the day. We didn't have a car or a phone anyway so I presume my parents must have walked to the nearest phone box to call the doctor when we were ill and he would always turn up at some time that day. I don't think anyone just turned up at casualty, they called a doctor first and if necessary the doctor called an ambulance.

These days everyone turns up at a surgery spreading their illnesses and germs to all and sundry. Sick kids are dragged kicking and screaming to the surgery when they should really be in bed. Half the doctors are sick because they catch everything going and never see the light of day so they spread it to the next victim..er...patient...and so it goes on. I am surprised we aren't all dying by the bucket load.

The problem is that we are not dying by the bucket load. It’s folk living longer that are part of the the problem because people are living longer in frail health instead of dropping dead. Then I think plenty of folk in poor circumstances are sitting in a warm doctor’s waiting room rather than in their own cold homes hoping for the doctor to magic their miserable lives into something better.

If you want to lump together poor health service, shortage of doctors & resources & everything else bad about this country then it’s a failure of successive governments.

That is indeed the reason for today's mess.

When I was young there was no such thing as a GPs appointment. If you wanted to see a doctor you just turned up at the surgery and waited. You were given a coloured disk with a number on. The number was your turn, and the colour indicated the doctor. The snag was that you could be waiting 3 hours and wouldn't even get a seat for the first 2. Then they introduced the appointment system, which usually meant a wait of no more than 10 minutes, if the doctor was running late. If you rang up before 10am you would get an appointment that day. If you rang after 10, you may still get an appointment that day if they weren't too busy, but in any case you would be seen within 24 hours. Whatever happened?

Colin

I started ringing my doctors at 8 o'clock this morning, got through at 20 past 8 and had an appointment for 20 to 9.

That's amazing! Haven't been able to do that with any certainty for the past 30+ years! On odd occasions we might be lucky, but not normally.

We have a few appointments ( face to face and telephone ) that we can book in advance, but most of our appointments can only be booked on the day. If the doctor wants you back they book it themselves.
It works very well.

I suppose that might work ok in some areas, but around here many people would never be able to get an appointment at all. A 3 to 4 week wait is average. You can only get an appointment same day if you can convince them it's very urgent, which means just short of a 999 call! Then they'll give you a telephone appointment. If they feel they need to actually see you, they will ask you to come in.
