Not quite sure if 'need' is the right word David. It's insurance, something you may require to make use of but hope you won't. At the moment we don't, fingers crossed. The way things are going, it might be made compulsory in the future, who knows?
Much is said about the NHS waiting lists but; we have a friend who was declared to require 2 new hips back in July. She will be getting the first in 2 weeks time - barely 4 months after diagnosis. And heres the thing, she could have had it in October, but they have been in Spain 3 months, returning only yesterday.

It also depends upon your personal situation. Mine was rushed through within 2 weeks. It was an extremely complicated replacement as cancer had eaten away the socket. Under normal circumstances I wouldn’t have been eligible for a replacement due to my bmi but in this case it was the only option open to get me walking again as well as getting rid of the cancer in that area
