3 way rocker switch is caravan/car / off
Does that go on car or caravan?
Secondly does the main charging switch need to be on to enable car battery to charge the caravan battery?

The car/caravan switch is a different circuit to the one that does the charging of the battery (and work the fridge) when the car is towing the van. So it does not matter where that car/caravan switch is set, but generally it is left in the middle position which is Off.
Same with the main charging switch which controls the charging of the battery when you are on Mains Hook-up (EHU).
Ofcourse all this is dependant on the fact that the caravan wiring is "as original" and has not been mucked about by a previous owner.
You can forget all the switches, providing that the car is correctly set up for charging, the battery should be charged when the grey lead is plugged in and the car engine running.

For a licence dated 1997 or later you must add together the plated max weight of the caravan and trailer, if the total is 3500 or less you can tow it. You may even tow a caravan with a MAM greater than the cars unladen mass the restriction was removed in 2013

Yes.
The 12v switch on the fridge usually has a light inside it. When it is switched to 12v, nothing will happen but when the car's engine is running and the grey plug connected, once power starts to come through to the fridge, the switch will illuminate to show that the fridge is running on 12v.
