Going back to the first post of the OP.

The metal prong overheated and the insulation on the (Neutral Blue) wire melted.

I suggest that would indicate a high resistance at the metal prong. Maybe dirty, or the socket connector was loose.

The socket on the van needs to be investigated for possible replacement.
