Have you tried asking reception what exactly is it they need?
A risk-assessment is exactly what it says on the tin : IS there a risk? If so, WHAT is the risk? and HOW will I remove this risk?.
As far as I am aware, there is no set form, it is usually a written statement of the above.
So in your case, f the site have asked you to carry out a Covid risk assessment, you probably need to state how you will make sure the van is cleaned before and after renting, how you will make sure that your renters have been informed of what they need to do to avoid either becoming infected or infecting anyone else. If your park has put restrictions on the number of people using any of the facilities, people visiting others on site, etc., etc., you will need to say how you will instruct your renters to comply.
