ACSI Campinggids Europa 2018 + App has the discount card, comes in 2 volumes and contains details of the 8200 ACSI inspected campsites in 29 European countries, SOME of which give discount with the ACSI card. You also get the App with this BUT it is available only in Dutch.

we have the complete book from 2011, that's in Dutch or German. We where still using it last year just thought it was time for an update as we have a campervan now & getting about a lot more.
