You have to pay the hospital anyway and then claim afterwards. Ask the hospital for the address of the local  CPAM (Caisse Primaire d'Assurance Maladie). You need to send a completed form (feuille de soins) which is usually the reciept from the hospital. You may be able to claim for the £20 as well as the £45, however for a grand total of £65 I do wonder if it is worth it!

My lad lost a finger nail whilst camping in Italy a few years ago.  Took him to Casualty with his E111, didn't pay a thing.  The finger needed to be re-dressed three days later, we were in Germany, took him to the site's local GP, showed his E111 still paid nothing.

A few weeks ago in Spain my wife came out in a rash, because of her warfarin we first of all had to go and get it checked at a clinic.Back to hospital and for 3 days running had to go back for injections in her BUM. Showed our E111 card and passport and paid only for the pills at the chemist.

My husband broke his leg when we were in France a few years ago. No one was interested in the E111 - they said we would have to go to the social security office but that was an hour's drive away so we didn't bother. We had to pay for everything up front  - even had to go to a pharmacy to buy crutches! We just put it all on the credit card (it was several hundred pounds) and claimed it back on insurance when we got home. I suppose we could have tried doing an E111 claim by post then but it was easier and quicker just to pay the insurance excess. The insurance company were excellent and even paid for the hotel we stayed in on the way home. It showed me how important travel insurance is!

Thanks for all your replies - most of you seem to have had an easier time than us.
Update so far - The Dept of work and pensions in Newcatle handle E111 claims.
It goes like this, phone french embassy to translate euro bill into pounds, send eng cheque in pounds to french hospital, when i get receipt from french hospital phone DWP and they will arrange refund over phone!
seems bit long winded, will let you all know how it goes!
I love red tape, me!!
