It all depends on how much fresh air you like. In the winter we have the kitchen window and the head and foot of the bed open (we like it fresh!) unless weather (mainly wind) dictates otherwise. At the very minimum all windows are on ventilation setting in winter.
In the summer we sleep with the van door open (with a porch awning). It's no different to sleeping in a tent, really then, plus any other ventilation as necessary. In really hot weather we have pretty much everything open.
