The only aspect I find puzzling is them entering the motorway down an exit slip road. To have entered at J8 they first would have to travel down the wrong side of a dual carriageway to have used that exit slip road and J 8a the occupants would have to have approached the exit slip road from the wrong side, of a roundabout as the exit is 'turned' left so would have been a tricky manoeuvre to have come around the bottom corner and then travelled up the exit slip road against the flow of traffic.
I think rather than debate / speculate on the whys and wherefores of this terrible tragedy, we should let the poor unfortunate people who lost their lives rest in peace.

I completely agree. Speculation is not helpful. Let's not forget that three people have died here.

Speculation and debate is what airs risky situations, which in turn may help to prevent others making the same mistake. Disappointing though it may be, loss of lives in tragedies, and subsequent arguments over them, are very often the only times that authority sits up and takes notice.
This not being the first occurrence, perhaps the highways authority will look at more visibility of signage, at the very least, to deter drivers from wrongly entering the exits of M-way slip roads.

The end of the slip road could be angled to the left before actually joining any junction or roundabout. That would make accidentally turning into the slip road in the wrong direction virtually impossible. It could be done, but would cost money! Sadly, the government will take into account the frequency of such incidents, and compare the actual cost of those accidents against the overall cost of making the road changes. And the cheapest will be the deciding factor.
