You're not using the wrong size pipes on the outlets are you?
If not, is your pipe ribbed ? how about some easily bent wire wrapped around behind the ribs leaving an arm either side and do the same behind the waste connector, push the pipe in and twist the two sets of arms together - just a bit fiddly though!
Other than that next time you go to a dealer or accessory shop take you outlet with you and see if any pipe they sell is a better fit than what you have got.
I have only had a pipe fall out once when there wastemaster was too far under the van before any water was put through, but usually once they are in they stay in whether the wastmaster was under the van or beside it.
