It's almost a copycat of an incident on the M1 in Leicestershire. The driver going the wrong way was 84, and he hit a van in which the driver in his 30's died.
In both incidents, I find it puzzling that the innocent drivers did not see the oncoming vehicles and then take evasive action. There have been explanations for M-way crashes that happen because many drivers do not look far enough ahead - i.e., not looking much further ahead than the vehicle in front of them. This habit could explain the rogue car 'appearing' when reaction was already too late.

Who's to say they didn't take (unsuccessful) evasive action? Maybe they swerved into lane two... just as the oncoming car/caravan did the same!
Or a similar swerve, only to be side-swiped by an artic?
Or maybe attempted to take the bit of rough between tarmac and central reservation (As the dash-cam car did), only to flip over, without making contact with the car/caravan?
Unless there's been a news update, confirming head on collision(s), we're all guessing as to the circumstances. I strongly suspect the innocent victim may well have seen the oncoming car/caravan, and did take, or at least attempted to take, evasive action. With traffic one side, and steel barriers the other (assuming all were still in lane 3 at this stage?) maybe there was no evasive action to take, worthy of the name! Not all such attempts can be successful, otherwise there'd be no accidents!
I agree that many "don't look far enough ahead", but even that is no guarantee of getting home for tea every night!
